Infertility - Diagnosis and Treatment

Before starting infertility treatments such as IVF or inseminations (IUI), you will need to undergo a series of diagnostic tests. In vitro fertilization will require more testing then would be needed for ovulation induction with Clomid. In Vitro Fertilization

IVF involves the stimulation of follicles with gonadotropins combined with ultrasound monitoring. HCG is used to prepare the oocytes for retrieval. Retrieval is performed by transvaginal needle aspiration before the oocytes are released into the abdomen. The oocytes are then combined with the sperm so that fertilization may occur. Techniques such as ICSI and assisted hatching require the IVF process. Patients with tubal factor infertility or male factor infertility will need IVF if the abnormalities are severe enough to preclude COH. ICSI is indicated when there are not enough normal, motile sperm for intrauterine insemination or standard IVF. It is also used frequently when there are poor fertilization results in previous IVF cycles. It is a micromanipulation technique originally developed in Belgium. By using a powerful microscope we are able to isolate a single sperm, aspirate it into an extremely thin glass pipette and inject it into a single egg. This technique does not require large numbers of motile sperm and bypasses the need for the sperm to penetrate the egg by itself. With the help of ICSI, fertilization is achieved and embryos that continue to divide are ready for transfer into the uterus in several days. Preimplantation genetic diagnosis is an advanced reproductive technology in which a single cell, or blastomere, is removed from the embryo and evaluated by genetic testing for various reasons. The most common reason is for the prevention of genetic diseases, which may be president in the embryo either from spontaneous mutations or from disorders inherited from the parents. The procedure involves making a small hole in the outer shell, or zona, using an acid solution or a laser with the assistance of aspecialized microscope. A small micromanipulation pipet is then used to gently pull out a single cell, which can be used for genetic testing.
